Insurance Policy

We are a participating provider for some of the plans offered by the following insurers; United Concordia, Delta Dental, PA Chip and Blue Cross Dental. For all other insurance plans, we request that you assign benefits payable to us and we will electronically submit and monitor your claim for payment. We will then bill you for any deductible or co-payment and the amount not covered by the plan. PLEASE REMEMBER…..your insurance contract is an agreement between you and the insurance company. The type of benefits provided for you depends on the contract that your employer elected to purchase for you. Insurance may pay all, some or none of your claim and you would be responsible for the difference.

Financial Policy

Payment is expected at the time of service. We accept CASH, personal checks, Visa, MasterCard, American Express, Discover, and debit cards. We do not offer in-house payment plans but can assist you in applying for credit with CareCredit.
